PROGRAM REGISTRATION
To register, please complete the Online Registration below OR you can download the ActiveChefs Program Registration Form here.

IMPORTANT! Enrollment confirmations will be sent as soon as the registration process is complete. We have experienced an increasing demand for our programs which are oversubscribed so only those families/children that need to attend will be notified.
For program inquiries or if you need help, please contact us directly. 

    Online Registration

    Parent/legal guardian MUST read, complete and sign/authorize. Please complete a separate form for each program registration and for each child participant.
    List any food-product allergies, dietary restrictions or medical condition that we should be aware of? * Please be advised that not all of our host venues / partner locations are an allergy-free environment *
    *All communications and confirmations will be sent to your e-mail address* We will need volunteers during the program, please let us know if you'd like to help out.
    In the unlikely event of an emergency, if you cannot be reached, please authorize an emergency contact person to act on your behalf, and notify him/her that he/she has been granted this authority and may be contacted by ActiveChefs.
